    My insertional Achilles tendon issue came from constant irritation from my figure skate pushing on my heel. Stopped wearing my skate 6 months ago, went through PT twice, heel lifts, orthotics, anti inflammatory meds, stopped running, skating, and ballet. 11 months of this issue, i am having surgery in July!

      Yes, it is actually very common to have both at the same time because it is the same type of activities or triggers that cause both.

      Usually for a few weeks - you want to be pain free while using them and built strength to that level before you start reducing the height slowly, week by week.

    Have you had any results using peptides? Pbc 157 and TB 500?

      We don't advise anyone to use those peptides since they have not yet been shown safe for humans to use and the research people quote to show their usefulness is from animal studies. And even in the animal studies, there isn't enough research to show they are safe. You will also find that the people promoting these peptides usually also promote other substances like SARMS which has been shown to be toxic to humans. I don't see the point in taking a substance that may or may not shorten your rehab by a month if it then messes up your kidneys or liver or something else.

    For insertional pain, at what week should you start stretching it again? Would ice help in the early weeks or would overall just not stretching it speed up the recovery?

      I usually advise my patients to refrain from passive stretches until we've restored the load bearing capacity (single heel raises with added weights) and also managed to transition to doing them over shallow step.     Hi. I switched to a barefoot running shoe in January and immediately noticed a big difference for my toes. I have Freiburg's disease which effects the blood flow to my metatarsals and I've had much less pain since then. But on the last 3 weeks I've been experiencing bad pain in my Achilles tendons. Sometimes one side, sometimes the other, or sometimes both. It's hindering my performance. I'm not a big runner. I do CrossFit and usually only run around 1-2km at a time, around 3-4 times a week. Do you think that this recent issue could be caused by the shoes? Until that point, I hadn't had any issues in either foot with the Achilles.

    Hello! I really appreciate your content. Your videos have helped me get rid of my years long symptoms from achilles tendinopathy. Unfortunately, 5 days ago I aggravated it while slightly had to walk faster. It was pretty sore afterward. However, 3 days ago while carefully walking around a park I once again went to walk a bit faster without thinking and felt a little pop in my achilles. Fortunately it did not rupture. The tendon is still attached. But now it hurts worst than before and it is painful to to push off from my toes from a standing position. Oddly, I can still do seated calf raises with some weight. My question is, I have noticed you don't mention wearing a boot for partial tears. Do you find that heel lifts are enough support the healing phase of a partial tear? Right now heel lifts are all I am doing for support.

      For less severe partial tears, heel lifts are usually enough. It is just the more significant (affecting large part of tendon) tears that may benefit from a boot.

      Yes, you'll very likely be able to walk at that point - just not super far yet.

    I have insertional tendin rupture from Moto accedent and i had surgery of 5 weeks with a cast as doctors said in the aurgery they used Screw supporter that had attached on the insertional tendon and and heel bone , so i am a remote area no physiotherapist, some one could support the next protocol and the physiotherapy needs , i am so appreciated if would help that matters????

    Hey, so I haven't had money to get a session with you guys, but my foot bursitis calmed down. Thanks for your video. I have one other question if once I start increasing load exercises, can it go away permanently. I'm still scared to do anything right now besides basic mobility training so it doesn't get stiff. There aren't enough videos, and the paper reviewed paper I tried to read costs 20-40$. Thank you again. Im just discouraged if I will be ever able to do high impact things again even if I get the muscles and tendons around my foot stronger. I will soon be seeing a sports physical therapist soon just got my insurance card.

      Your rehab will slowly help you restore your foot and bursa's capacity to do your exercise and training - as long as you slowly ease into things, it will be OK. Also, try to identify why it happened in the first place, so you can avoid making the same mistakes in future.

    My mistake was to run at exactly the same Easy/z2 pace when returned to running which trigerred a flare-up and a big setback. The key is to go zone 1 or even slower and monitor the morning pain/stiffness in the 24hr recovery cycle

      Yes - good advice - forget what used to be easy for you when you weren't injured and fit - test what your tendon can now cope with by doing much less at first than what you think you should.

    Great video and explanation of heel bursitis. It is exactly what I have experienced for months - it was caused and exascerbated at work by the constantly getting on and off a tug hundreds of times a day to load and unload oversize items onto various carousels at the airport. The heel stretches I have been doing were not helping so I will cease these until the bursitis is better. Many many thanks.

    That was a great video with very helpful information. I was attempting to diagnose the pain I have been feeling and there was no difference between pinching the Achilles on either side. However when I lightly apply a massage gun (very lightly) to back of the tendon there is a decent amount of pain. Zero pain on the other side. This is an older overuse injury from hiking too much and continues to come back. It fits most of the descriptions for tenonitis, but I'm curious if you think it could be something different given the self diagnosis technique?

      Hard to say - but sometimes the injured area is not where it can easily be pinched. So if the rest of the history for how it started and how the symptoms now reacts to different activities fits, then it might be tendinopathy.

      It depends on how much walking, but you can usually start placing weight through it at around 3 months post op and walk normally by 6 months if rehab goes smoothly.

      It varies depending on the person but it usually takes about 6 to 9 months to get back to full function. For some, it can take longer.

    I've been looking around for what to do after an Achilles Tendon rupture reconstruction. This is by far the best one yet. All in one video and not divided into too many videos. I understand some videos were more detailed about each week. But they're more specific for that patient's case. You explained for a multitude of cases 🤣 Which answered all I needed to know. I'm just about 2+ weeks post surgery. They'll put me in a Boot probably by week 3 or 4. Since my surgery was a bit complicated since mine was a Neglected Achilles Tendon rupture lol 🤣 I've already set that I won't be doing hard sports for 6 months and would probably be just doing light work for 4-9 months. Definitely set no dorsiflexion until month 6 then 🤣 But we'll see how things progress. Glad you included it at the 12+ week mark 🤣 Something to look forward to every week. Thanks again for the video.     Very many thanks Alison for your most welcome reminder of the importance of progressing my lengthy exercise programme to return to comfortable walking. My left heel tendonosis came completely out of the blue but as a 78 year old, 70 Kg male, after 18 months of benefitting well from your invaluable videos my recovery had frustratingly plateaued at I would guess about 85%. But after your reminder I firstly added one 4Kg then after 3 weeks two 4Kg weights to my backpack before doing my routine single leg heel raises and progress was thankfully resumed... It's probably now nearing 98% - probably even better if I had the courage to return to the longer distance walking I had previously taken in my stride for many years!

    I dont run or jog, yet ive have archilles pain in heel several times a year.nit just comes on and pain lets up in a week. The painnis awful like a knife stuck in back of heel. I stretch and ice but nothing, take advil ..nothing.

      You can get if from walking and even just the type of shoe you wear - but you may also want to get it checked for gout - gout can cause ++ pain and act like Achilles tendonitis.     Thank you for the information!😁 Also, as a footballer, once I hit something like 2-3 sets of 60s side to side single leg pogos would I be ready to sprint and even play once again? If, due to a lot of activity a few days ago, my Achilles starts being just a little bit painful, should I rest for the rest of the week or just avoid my plyos?

      You usually don't have to rest completely but you must reduce intensity of training for a few days until it settled down and then ease back in. I can't really make an informed recommendation for when you're ready to play football without knowing your full situation but it does sound reasonable that if you can do that amount of plyo work, you might be ready to ease into training. But remember, you have to build up running from jogging slowly over time to sprinting and change of direction - you can't just jump directly into that.

    I really appreciate your videos. I have been struggling with achilles tendinopathy since December. Still not feeling I can get back to running. Have had a session with one of the physios at treat my achilles. What I can't manage is getting the strength training right to get me back to running. Also very interested to know what your opinion is of the compression boots for achilles recovery? Thanks

    In the study, what is the "slowed down" group, and what is the "traditional" group mean? I was assuming traditional was going slower with the weight bearing, etc., but slowed down sounds the same.

      The traditional group started stretching into dorsiflexion early in rehab and the slowed down group only started after 12 weeks. Both groups started weightbearing at the same time.

    How different would recovery from a slightly different procedure look: Haglund’s recision, removal of bone spurring and retrocalcaneal bursitis, and debridement and reattachment of the Achilles (no rupture/tear)? Safe to assume that since no rupture, the recovery process will proceed a bit more quickly?

      It depends on your surgeon's protocol - they vary quite a lot, but yes, people tend to get active more quickly. However, in my experience, if you introduce super strong stretches too early, it often irritates and causes +++ pain. So I am a fan of gentle and often rather than super strong for that scenario.

    Thank you! I just had left heel surgery for Haglunds Deformity / Insertional Achilles Tendinosis. I am a 65 yr old athletic person. This gives me a solid understanding of what I should expect regarding PT and expectations moving forward. My mind wants to rush the process, but your explanation was VERY clear on why I need to “pump the brakes” and be patient for long term results. Thank you again!

    hii , how come numbness shoots through the outer side of my achilles tendon on touch ?

      Those types of sensations are usually due to nerve irritation - so in your case something must have irritated the nerve that supplies that area.
